Cow in the ditch

Problems and failures are normal. But whenever you are struck in any problem, you need to do three things:
1. First, get the cow out of the ditch
2. Second, find out how the cow got into the ditch
3. Third, make sure you do whatever it takes so the cow doesn’t go into the ditch again.
